Independent Contract Income
If you select Independent Contracts as your Acquisition Method, you must provide at least one of the following documents:
- Form 1099 (Copy B)
- Form 1099-K
- Form 1099-NEC
- Form 1099-MISC
- Form 1040 Schedule C
- Form 1120
The document submitted must be valid and clearly show your independent contractor or business income.
Small Business Owners
If you select Small Business as your Line of Business, you must provide documentation that verifies the existence and ownership of your business. You may submit one or more of the following documents:
- Certificate of Organization
- Articles of Incorporation
- EIN Letter
- Operating Agreement or Partnership Agreement
- State Entity Search Result
- Fictitious Name (DBA) Certificate
The Small Business option is intended exclusively for business owners. The documentation submitted must verify the legal existence of your business.
